Personal Data Collection Statement

Dim Sum Music takes personal privacy very seriously and ensures that all procedures for collecting, using, storing, transferring, and accessing personal data under any circumstances comply with the requirements of Hong Kong's Personal Data (Privacy) Ordinance. We will ensure that our staff strictly adhere to this Personal Data Collection Statement and our Privacy Statement. Please read the following carefully to understand our Personal Data Collection Policy. We will not provide your personal data to any third party for any purpose without your consent.

收集資料的目的

點心音樂會使用你提供的個人資料，向申請人提供申請人所需要的活動、課程或服務，包括參與活動、課程或服務的相關用途、簽發收據、收集意見及資料分析。同時亦會使用申請人提供的個人資料向申請人提供有關點心音樂及有關單位的相關活動、課程、服務及捐獻要求等推廣資訊，申請人可隨時向點心音樂表明或更改接收推廣及宣傳意願。閣下有責任提供申請表格上列為「必填」的資料，或啟動相關流程必須提供的資料，否則有可能無法提供閣下要求之服務。

Purpose of Data Collection

Dim Sum Music will use the personal data you provide to offer applicants access to the activities, courses, or services they require, including the intended use of the activities, courses, or services, issuing receipts, collecting feedback, and data analysis. The personal data will also be used to provide applicants with promotional information regarding Dim Sum Music and related organizations' activities, courses, services, and donation requirements. Applicants may indicate or change their willingness to receive promotion and publicity at any time. It is your responsibility to provide the information listed as "required" on the application form, or any information necessary to initiate the relevant process; otherwise, the services you request may not be provided.

 

資料的轉移

申請人所提供的個人資料，會供點心音樂在工作上有需要知道該等資料的職員或指定人士使用。不會租用、出售、轉移或披露所持有之個人資料予他人或非有關單位，除非：i. 對方為於業務上向點心音樂提供服務的代理機構、承辦商或服務提供者；ii. 閣下要求之服務由有關人士／公司負責提供；iii. 已預先得到資料當事人的同意；iv. 對非法活動、懷疑詐騙、涉及或威脅到任何人的人身安全的事件作出調查、預防及採取行動；v. 遵循所有適用法津、規定、法律程序、具法律效力的政府要求、行政制度或規例要求。

Data Transfer

Personal data provided by applicants will be used by Dim Sum Music staff or designated individuals who have a need to know such data for their professional purposes. Personal data held by Dim Sum Music will not be rented, sold, transferred, or disclosed to any other person or entity except: i. to an agent, contractor, or service provider providing services to Dim Sum Music in connection with its business; ii. to the relevant individual/company providing the services you requested; iii. with the prior consent of the data subject; iv. to investigate, prevent, or take action regarding illegal activities, suspected fraud, or incidents involving or threatening the personal safety of any person; and v. to comply with any applicable law, requirement, legal process, legally binding governmental request, administrative system, or regulatory requirement.

 

查閱及更改個人資料

根據該條例，閣下有權查詢我們是否持有閣下的個人資料，查閱我們記存的閣下個人資料，要求我們更正所記存不正確的個人資料。

申請人請確保向點心音樂提供的資料正確無誤，並有責任向點心音樂更新資料，否則有可能無法提供閣下要求之服務。申請人有權要求查閱和改正所提供的個人資料及索取有關資料的複本。如需查詢或改正其個人資料，可填妥個人資料私隱專員公署的查閱資料要求表格 (OPS003) (https://www.pcpd.org.hk/chinese/publications/files/Dformc.pdf)，或以書面形式申請，並電郵至 info@dimsummusic.com 。點心音樂在處理查閱或改正資料要求時，會查核提出要求者的身份，以確保他／她在法律上有權作出這項要求。在收到閣下提出的要求後將在 40 天內給予回覆，點心音樂將可能就此收取合理的費用；點心音樂須在或可在《個人資料(私隱)修例》第 20 條指明的情況下拒絕依從查閱資料要求，例如申請人未能提供點心音樂合理要求的資訊。

Access and Correction of Personal Data

Under the Ordinance, you have the right to inquire whether we hold your personal data, to access your personal data held by us, and to request that we correct any inaccurate personal data held by us.

Applicants are requested to ensure that the information they provide to Dim Sum Music is accurate and are responsible for updating it. Otherwise, Dim Sum Music may not be able to provide the services you request. Applicants have the right to request access to, correction of, and a copy of the personal data they provide. To request access to or correction of their personal data, they may complete the Data Access Request Form (OPS003) issued by the Office of the Privacy Commissioner for Personal Data (https://www.pcpd.org.hk/chinese/publications/files/Dformc.pdf) or submit a written request by email to info@dimsummusic.com. When processing a data access or correction request, Dim Sum Music will verify the identity of the requester to ensure that he/she is legally entitled to make the request. Dim Sum Music will respond to your request within 40 days of receipt. Dim Sum Music may charge a reasonable fee for this. Dim Sum Music must or may refuse to comply with a data access request in the circumstances specified in section 20 of the Personal Data (Privacy) Amendment Ordinance, for example, if the applicant fails to provide the information reasonably requested by Dim Sum Music.
